Vendor Profile


The Office of Diversity & Inclusion maintains databases of vendors for goods/services and construction.  We use the information to share upcoming construction project information as well as provide schools and departments with a list of vendors for various goods and services.  

If your firm would like to be included in the database(s), please complete a CMS Vendor Profile Form by clicking the following link:  CMS Vendor Profile

The form will open as a fillable PDF document in your internet browser.  Once the form is completed, please save and email to the appropriate contact listed on the form.  You will be notified once your application is processed.

Procurement Services Vendor Application


If your firm is already working directly with a school or department or they have indicated an intent to use your firm, there is a separate application to complete. Procurement Services requires all vendors to complete an online Vendor Application and submit a W9 in order for CMS to issue a purchase order and remit payment (Net 30 payment terms).  The online Vendor Application can be accessed using the following link:  Online Vendor Application
