to access the guardian RHASE opt out forms for middle and high school students (grades 6-9)

to access the guardian RHASE opt forms for 5th grade students.
Forms are in English and Spanish and must be sent out two weeks prior to instruction. These forms are passive permission, meaning if the form is not returned to the teacher, the student is automatically allowed to receive RHASE instruction. The only way a student can opt-out of RHASE is having a guardian complete a RHASE opt-out form, checking the box, "I do not give permission for my son/daughter to participate in the Reproductive Health and Safety Education unit."
